Documents to Bring:

• Current CPR & First Aid Certificate

• 2-Step MANTOUX TB Skin Test or Tuberculosis (TB) Screening Evaluation Report

  • (History of positive TB only)

• ID's

  • State ID or Valid Driver's License

  • SS Card

  • Alien Card (if not a U.S. Citizen)

• High School Diploma / GED / College Diploma

• Hawaii Nursing License (RN, LPN, CNA)

• Field print - FBI Fingerprinting
