I/DD Medicaid Waiver Services

We support individuals with intellectual and developmental disabilities (I/DD) through Medicaid-funded services. Our person-centered planning process ensures that each individual's needs are met, helping them develop daily living skills, personal care, social skills, and community involvement. We also offer professional nursing care to provide respite for caregivers.

What is ResHab?

Our Residential Habilitation Services, commonly known as ResHab, provide comprehensive assistance and training. These services are not limited to activities of daily living and independent living skills but also extend to engaging in typical household activities, family functions, community events, and social and leisure skill development. This holistic approach ensures that participants receive the support they need.

Our dedicated team is committed to helping them develop and maintain daily living skills in a nurturing residential environment. We focus on personal care, social skills, and community integration, ensuring each individual can lead a fulfilling life.

Residential Habilitation (ResHab)

What is PAB?

Personal Assistance/Habilitation (PAB) consists of a range of assistance or training to enable participants to acquire, retain, or improve skills related to living in the family home or their own home. PAB services may include assistance and training in activities of daily living such as bathing or dressing, independent living skills such as laundry or cooking, and communication and interpersonal skills.

Our one-on-one support for individuals with I/DD is far from a one-size-fits-all approach. We understand that each individual has unique needs and tailor our assistance accordingly. Our goal is to help them navigate daily activities while fostering their independence. Our commitment is to empower them to thrive in their everyday lives, building confidence and promoting personal growth.

Personal Assistance Habilitation (PAB)

What is Respite?

Respite and Respite RN/LPN services offer short-term care for participants residing in family homes. These services relieve those who typically provide unpaid care, covering at least a portion of the day. These services can be provided in various locations. If nursing assessment, judgment, and skilled interventions are required during respite, the service may be delivered by an RN or LPN under the supervision of an RN.

Caregiving can be both rewarding and demanding. Our professional nursing care offers temporary relief for primary caregivers, ensuring quality care for their dependents while they take some time for themselves. This service provides temporary care for individuals with I/DD, allowing primary caregivers to rest and recharge. It recognizes the importance of caregiver breaks and supports loved ones during these times, providing relief, peace of mind, and a sense of being supported.

What is CLS?

Community Learning Services (CLS) are provided in the community to support participants in improving their access to the community by enhancing their skills, improving communication, developing and maintaining friendships, and gaining experience with available opportunities.

Our personalized program supports individuals with I/DD in engaging in community activities and developing essential social and vocational skills. We aim to encourage their growth and facilitate meaningful connections within the community.

Community Learning Services (CLS)

What is Chore?

Chore services are essential for maintaining a clean, sanitary, safe home environment. These services are available to participants living in their homes and or those without natural (non-paid) support or with family members who cannot perform the tasks.

We acknowledge the significance of community involvement and belonging. Our personalized program supports individuals with I/DD in engaging in community activities and developing essential social and vocational skills. We aim to encourage their growth and facilitate meaningful connections within the community.
